San Nurse Assess

A native iOS and Android app that lets home health nurses see their day, intake patients and sign forms in the field, cutting intake from 15–20 minutes on paper to 3–5.

Overview

Nurses intake patients in the field, often on tight schedules. This native iOS and Android app shows each nurse the patients scheduled for the day and lets them complete and sign patient information on the spot. Speed in the field drove every decision. I was lead UX for research, design and user testing, working with clients, subject matter experts and a business analyst to capture the requirements.

Key outcomes

3–5 min intake

3–5 min intake

Average patient intake by a nurse, down from 15–20 minutes on paper

5 sec to the next route

5 sec to the next route

Down from 5–10 minutes of looking up addresses and planning routes by hand

Pain points

Paper forms

Nurses filled out patient documents by hand, on paper.

Information overload

Nurses had to work through too much unnecessary information.

Routes planned by hand

Getting from patient to patient took 5–10 minutes of looking up addresses and planning routes.

Requirements

  • Portable

A portable application for nurses on the go.

  • Patient intake

    Basic patient intake.

  • Daily schedule

A list of the day's scheduled patients.

Information architecture

  • I designed the app from the ground up. Observing nurses at work showed me their process and the information they need to capture
  • Nurses see at most eight new patients a day, so AI orders them by the most efficient route
  • Each day opens on assigned patients, with quick access to visits started but not submitted, and to all patients
  • Patient forms adapt to medical history and other criteria. I reviewed real cases with nurses, captured the logic behind the most common ones and built it into the system, removing a lot of unnecessary information
  • I set up the dynamic rules, groupings and permutations with the backend and database teams and a business analyst

Innovations

  • Smart routing

Nurses don't have to plan their day's route. Next on the roadmap: opening each stop in Google or Apple Maps straight from the app

  • Smart forms

Dynamic forms tailored to each patient
